There are two ways to make a claim:
Claim Procedure - Guarantee
A. Inpatient Claims History
1. Make sure you contact the Medical Services 24 hours PRUhospital & Surgical 75 done prior to hospitalization, or a maximum of 2 x 24 hours after hospitalization is done (if an emergency).
2. Clarification insured yourself will first be done by the Medical Services Officer 24 hours. The information will be asked include:
- Your name (the insured card holder PRUhospital & Surgical 75)
- Phone number
- Policy Number
- Date of birth
- Name of Hospitals and Doctors who want to target (if any)
- Referral letter from doctor
- Symptoms or medical conditions that you face that require hospitalization
3. The next 24 hours Medical Service Officers can provide references or information on the Hospital who became a partner / provider or you can choose the hospital in accordance with your wishes as long as the list of providers. The waiter Medical Officers can provide 24 hour medical advice if you need
4. As soon as possible after you arrive at the Hospital. You must show your card to an officer of insured hospital administration.
5. During your treatment and comply with the maximum benefit shown on your policy, you do not have to worry about the cost of hospitalization. Because of the costs incurred in relation to treatment will be guaranteed or covered in advance by the PT Prudential Life Assurance. If there is any difference that exceeds the maximum benefit stated in the Policy, including if you are hospitalized in a room that costs above the limit of your policy benefits, then the difference between costs in question will be informed to you. B. Outpatient Claim (special care before and after hospitalization)
Insured Surgical PRUhospital Cards & 75 you can not be used as a guarantee of payment for outpatient expenses that you do. Outpatient claims, as stated in the Policy Summary and Policy Provisions, can you come up after treatment, with the following provisions:
1. Claim Form with a complete, clear and true. Forms can be obtained at the Prudential Life Assurance Office or through the website on the "Claim Form and Other Applications"
2. Completing the document persyatan claims, namely:
- All original invoices and receipts for the cost of care
- Full Report of Physician / SKD (Certificate of Physician)
- Details of the cost of care from doctors, including the cost of medicines and services rendered
